I've released a generic HTML form to email processing PHP script under the GPL. This script will process just about any form and send the results to a recipient of your choice. All configurable from within the HTML form itself using hidden inputs, so you can have one script that processes all forms on your domain.

In order to use php-form2mail you'll need to edit the form2mail-config.php file and set some defaults. Some of these control default behavior if it isn't set in the actual form (such as the recipient of the emails the script generates), some of it provides "screen door" security.
